Baptismal Certificate Request Form

Privacy Notice

Following a Catholic baptism, the following details are entered into the Catholic Baptism Register: the name of the person baptised and their date of birth, the names of the parents of the person baptised, the home address at the time of the baptism, the names of the Godparents and Christian Witnesses, the name of the officiating minister, the place and the date of the baptism.

After the baptism further sacramental information, such as confirmation and marriage details (where applicable), are added to the entry once these sacraments have been received. Confirmation details include: place and date of reception, and the name of the officiating minister. Marriage details include: date and place of the wedding ceremony, and the name of the spouse.

The legal basis for retaining this information is that it is necessary in the legitimate interests of the Church to do so. Some of the information retained is special category information because it directly, or indirectly, reveals your religion. The retaining and processing of that personal data is a legitimate activity of the Church and relates to members or former members of the Church (or to persons who have regular contact with it) in connection with its purpose, this includes non-Catholic parents, and non-Catholics acting as Christian Witnesses. The reason for retaining the information is that the Church needs to know who it has baptised, confirmed and married.

The privacy impact on you caused by this retention of your personal data is minimal because it is kept securely and will be provided only to the person whose baptism it was, or in the case of a minor, to their parents/legal guardians on proof of identity.
